Programme Description

Professor David Haslam, Chair of NICE, explains how to implement the recommendations of the NICE guideline on multimorbidity in primary care. The module outlines what’s new in the guidance, what to do differently in practice and how to improve quality of life and optimise care for patients with multimorbidity.

After taking this module, GPs and nurses will be better able to:
  • Recall what’s new and what’s relevant to them in the September 2016 NICE guidance on multimorbidity
  • Identify patients who need an approach to care that takes account of multimorbidity
  • Know how to assess frailty in primary care settings
  • Describe which factors to consider when agreeing a bespoke management plan with a patient with multimorbidity
  • List ways to improve the quality of life for patients with multimorbidity
  • Identify conversation triggers that might mean patients want to talk about ways to manage their multimorbidity
